I start with a background section, then list the materials reviewed and before I start listing my opinions and the bases therefor, I say this:

After listing each opinion and why I hold it, I may sum up with a brief conclusion, and to demonstrate that things may change, I add the following disclaimer:
The opinions expressed herein are based on the authorís education, training and experience, and on the materials reviewed to date. Should additional information come to light, the author reserves the right to amend or supplement this report.
The author has made every effort to accurately and completely investigate all areas of concern identified during this investigation that are relevant to the expressed opinions. If new data become available or there are perceived omissions or misstatements in this report, the author asks that they be brought to his attention as soon as possible so that he has the opportunity to fully address them.
