Yes, it was X-rayed. There is considerable mass loss at the center of the receptacle. Arc tracking due to moisture was considered and while not completely eliminated, the EEs considered this unlikely as the exterior rated extension cord was only several months old and used indoors. There was no evidence to support a hypothesis that the outlet was subject to wet conditions.
