I am a Heavy Equipment Specialist and see quite a few hoses that have failed in various ways when investigating equipment fires. I have puzzled over the same thing, but so far, the hoses seen have not been associated with the cause.
I can speculate several scenarios. First would be a BLEVE that occurs when liquid inside the hose is being heated and literally "blows" the end off of the hose. However, I have found some that appeared to have blown the ends off on both ends, which might have occurred simultaneously, but you would think one end would give up before the other and the pressure would then be relieved.

To be sure, hoses can blow the ends off with hydraulic pressure as well. The hose could have been assembled out of components that were of insufficient pressure rating. Hose assembly is a more likely suspect because the correct dies and components must be used together to obtain the correct crimp on the hose. Dies and assembly tools wear over time and can get to the point where they do not provide sufficient crimping force.
