When you perceive that the document that serves as the de facto standard of care for your profession contains errors, it is your professional duty to point those out using the process designed for that purpose. Pointing out errors in this forum, or vaguely referring to other unspecified "unfortunate inconsistencies," does not fulfill thus duty.
I look forward to reviewing your proposals for changes to correct those perceived errors. The document will not become the professional error-free treatise that you wish for unless you help to make it so.
