Mike:
Try the International Fire Service Training Association. Their documents clearly define what is "Salvage & Overhaul" and how it relates to Fire O & C.

You can also try page 176 of Kirk's Fire Investigation , 5th ed, under "Minimizing Post-Fire Damage"
More detailed information can be found in the following book relating to Public Fire Service Tactics: "Strategic And Tactical Considerations on the Fireground", Brady Books, by James Smith, pages 79, 134-142. ISBN: 0-13-061451-3
Chapter 3, entitled "Decision Making" states "Overhaul ensures that all fire has been extinguished. Areas where fire could extend must be opened and checked. Smoldering contents must be removed to the exterior. The overhaul stage is a critical step in complete extinguishment. When poor overhauling techniques are used, rekindle can occur."
Unfortunately, this document also states in the next paragraph, "...Overhauling operations also involve determining the cause of the fire. Telltale signs of origin and cause can be uncovered."
Hope this helps.
Chris
