The investigator or Fire Marshal determine the classification of a fire. The claims manager determines whether the circumstances surrounding the incident meet the company's contractual basis for payment of the claim. They are not the same factors.
Yes, it's certainly possible for a fire to be classified as undetermined (or even accidental) and the claim still be denied for other valid reasons. However, in many jurisdictions the company has to provide a reason for the denial.
