I can't answer your questions because I destroyed my notes as is my regular policy when dealing with an argument that argumenative persons are not. But you can take my word for it because you should give me the benefit of the doubt and consider me innocent (read truthful and not mistaken) until proven guilty (read a liar or an idiot).
It is my policy to destroy notes before I read them because they are too big to store in the file, they got dirty at the fire scene, they might be pathologically unsafe, they might prove me wrong, or most importantly: because it is my policy.
It is my policy to destroy notes because that is my policy. In fact I never ever take notes and later I destroy them anyway.
You can take the last word on the subjects of destroying notes, "standard of care" etc., because this is mine.
