Prior to the introduction of compressed air for breathing apparatus cylinders, medicinal oxygen was used. The very first thing we were taught as breathing apparatus wearers ( and we trained to do our own apparatus servicing and maintenance) was: "before touching any of the components you must thoroughly was your hands to remove any grease or oil residues including your own natural body greases."
It is conceivable that when changing the cylinder, oil or grease residue was on the hands of the person making that change. The "O" rings should always be inspected for damage when making such cylinder changes and that also leads to the potential for contamination.
