It would appear that some one during the testing of the regulator left oil in the regulator.
Everyone will deny that it could happen but it does and its all the time I have investigated a couple of these fires, Its the most Proable cause of the failure.. I would suggest taking the ASTM G-4 class on problems with oxygen fires.
