Just had another thought on this. It is conceivable that there was damage to the "O" ring so there was a potential for a leak from the cylinder to be ignited by static electricity.
Remember, hospitals have or had the copper or brass studs placed on gurney wheels to prevent such ignitions and hospital staff were required to wear cotton clothing.
Not all situations are possible to provide anti-static prevention, particularly in a fire department squad or rescue vehicle, especially when transporting patients. You never know what the clothing of the FD personnel or patient is made of!
